I need a specialized form that is not included in the CCH Form Library. How do I create a custom form in CCH Axcess Workstream?

  1. From the Dashboard, select Configuration > Lists from the Workstream section.
  2. Select Form Due Dates.
  3. Select the New button at the top left of the screen.
  4. Enter a Form ID and Name.
  5. Select if Date Basis is Fiscal Year End or Calendar Year End.
    • Note: Select the Frequency if form is Calendar Year End Enter Due Dates.
  6. Select Save and Close.
