This message is usually caused by an Internet connectivity issue.  Please close out of CCH Axcess Document and reopen it.  This will re-establish the connection.  Then attempt to do what you were doing in CCH Axcess Document when you received the message.

If the above does not fix the issue, open Document and click on the Recent Entities tab and remove all entities.  Close and re-open Document.
