Diagnostic 60638 - "State Electronic Filing. The preparer tax identification number (PTIN) must be confirmed to be accurate by the registered owner prior to electronically filing this return. This confirmation is missing in either Office Manager, Interview Form 3, Box [#], or Interview Form EF-2, Box [#]. The PTIN must be confirmed where it was originally entered in Office Manager or Tax Preparation."

Cause
This diagnostic issues when the PTIN information is entered, however, the registered owner of the PTIN has not confirmed that it is accurate.
Resolution
Depending on where the PTIN information was entered in (either in Office Manager or in Tax preparation), there are different locations the PTIN could be verified.
